2017 HFM Academic Scholarship Award:

 

The Foundation will be issuing two $2500 scholarships for the 2017 academic year.  All components of the application will be considered.  Applicants will be notified by April 5, 2016 if they have been chosen.  Scholarships will be presented to recipients during the SpringFest Conference held at the Eagle Crest Conference  Center in Ypsilanti, Michigan.

The scholarship applications are reviewed by a scholarship committee.  All applications are redacted of any identifying information before being given to the committee.  Each scholarship committee member uses the same rubric to score applications.

 

Scholarship Guidelines:

  • Anyone with a bleeding disorder may apply (clotting disorders are not eligible for this scholarship)
  • Immediate family members (parent, sibling, grandchild) of those with a bleeding disorder may apply
  • Michigan residents seeking a post-secondary education, which can include a community college, university, graduate level or trade school
  • Past applicants are eligible to re-apply
  • Previous scholarship recipients are not eligible to apply
  • Applications must be received electronically March 20, 2017
  • Transcripts must be received by March 20, 2017
  • Incomplete applications will not be considered

Scholarship Application Checklist:

 

1. Online Scholarship Application

  • To ensure confidentiality, redacted applications will be viewed by the Scholarship Review Committee only

 

2. Proof of Bleeding Disorder

  • Please upload a letter from a health professional verifying the bleeding disorder of the affected person. 

 

3. Goal Statement

  • Please describe how your bleeding disorder (or your relative’s bleeding disorder) impacted your educational and career goals? 
  • Please submit a two page response (documents must be a .doc/docx or pdf)

 

4. Two Reference Letters

  • Please upload two letters of recommendation (not related to the applicant)
    • Teacher, instructor, or professor, employer, Community Leader, or Health Care Provider, or other professional that knows you personally

 

5. Official High School Transcript

  • Please have an “official” transcript mailed to:

    • Faxed, electronic, or unofficial copies will not be accepted

 

6. Official College Transcript (if applicable)
